QUESTIONNAIRE ON YOUNG IMMIGRANTS’ BELIEFS ABOUT EXCLUSIVE TAP WATER CONSUMPTION (EDI)

Hi,

We are inviting you to complete an online questionnaire that lasts approximately 15 to 20 minutes. The questions will be about your beliefs on tap water consumption and include sociodemographic information. The data collected will be used to better understand factors that can influence tap water consumption among young immigrants and will eventually be used to develop interventions to encourage tap water intake among this population. This study is funded by VITAM – Centre de recherche en santé durable [Research center in sustainable health].

By participating in this research, you will be able to contribute to the advancement of knowledge on this topic. No other disadvantages than the time it takes to complete the questionnaire was identified. To thank you for your participation, you will be emailed a $20 electronic gift card for a sports store (Decathlon) at the latest one month after you completed the questionnaire. You are free to participate or not in this study.

Water, diversity and immigration: Study on young immigrants’ (14-17 years) beliefs on tap water consumption

Anonymity Measures:

The following measures will be applied to ensure that the information you provide remains anonymous:

  • Data will be collected confidentially.
  • At the end of the questionnaire, you will be asked to give your contact information in another form to send you the electronic $20 gift card for a sports store (Decathlon). Your contact information will not be associated with your responses to this questionnaire. Also, they will be deleted no later than three months after the end of the study.
  • Your name will not appear on any documents.
  • Your answers will never be shared with others.
  • All electronic documents of this study will be kept on the password-protected computer of the principal investigator for seven years and be destroyed afterwards.
  • The results will be published in a scientific journal and presented at scientific meetings, but it will not be possible to identify any participant.

If you decide to participate in this study, this means that you freely agree to participate according to the conditions mentioned in this document. You also free to withdraw at any time without prejudice and without to justify your decision.
